Chandra Smith, MSN, APRN, FNP-C is a board-certified Family Nurse Practitioner credentialed through the American Association of Nurse Practitioners, dually licensed in Illinois and Wisconsin. She received her Bachelors of Science in Nursing from Rockford University and later pursued her Masters of Science in Nursing from Olivet Nazarene University in Bourbonnais. Chandra has a diverse nursing background across multiple care settings and is well versed in integrative, regenerative and functional medicine, hospice and palliative care, acute hospital and long term care, school nursing and nursing education. She holds additional certifications in bioidentical hormone pellet therapy.
After spending a greater majority of her career working in palliative and hospice care, assisting patients with advanced symptom management and progressive life limiting illnesses, she felt compelled to change her focus towards preventative medicine. Chandra was introduced to functional medicine on her journey to solve her own health issues. It was only through alternative medicine that she was able to begin healing and discover her passion, later implementing these concepts into her own practice. She has gone on to pursue additional training in functional and holistic medication with a significant interest and personal connection with alternative cancer treatment, metabolic disorders, and infertility.
Our model connects traditional, preventative, functional and holistic medicine to meet the patient where they are on their path to health. Chandra strives to improve the quality of life for her patients while practicing with the utmost compassion and personalized approach to address each patient’s individualized needs, while making them feel heard.

Alicia Shelton, RN specializes in functional medicine. She is certified in bioidentical hormone replacement therapy and is a certified Lyme specialist with a focused interest in nutrition and toxin management. She was raised in the Rockford area. She started her nursing career in dialysis and then transitioned to hospice. After years of watching many patients suffer from kidney failure, cancer and dementia, she realized that she wanted to help on a deeper level, and focus on whole body wellness and preventative care. She then found her passion in integrative functional medicine.
She enjoys staying active by running, biking or chasing after my 2 young children. Her family has embraced living a wellness lifestyle and she would love to help you embrace yours.
