How it works:
Your name will be entered for each new patient you refer to Circle of Wellness for any service. That new patient also gets their name entered into the contest.
The person with the most referrals wins.
Winner does not have to be a
patient of Circle of Wellness.

Fine Print...but not too fine.
• Referrers may distribute these coupons with their contact information to referees to be presented at referee’s new patient appointment but referees need not present a coupon to be entered into the drawing.
• New patient must state referral’s full name and may be required to produce referral’s contact information if they are not in Circle of Wellness records.
• New patient Contest is only good for referrers and new patients referred between Dec 17th 2012 and Jan 31st 2013 - past referrals are not eligible.
• Winner will be based on number of referrals between Dec 17th 2012 and Jan 31st 2013.
• Winner will choose dates of travel. Trip must be booked within 90 days of winning. Travel dates must be taken within 12 months.
• Winner will choose hotel accommodations and airfare package not to exceed $845
• Winner need not be present on Feb 1st 2013 to win.
• Winner does not need to be a current or past patient at Circle of Wellness.
• ***Winner may redeem prize for cash, if they do not want the trip to Las Vegas
• In the event of a tie, those individuals will choose a number between one and 100 and whomever is closest to the preselected number will win.
